Lower back pain is the most common cause of disability worldwide. It affects roughly 80% of adults at some point in their lives, costs the U.S. healthcare system hundreds of billions of dollars annually, and is one of the leading reasons people miss work.

And yet, for all of that, the conventional medical approach to lower back pain has a remarkably poor track record.

Why Surgery Often Isn't the Answer

Back surgery is one of the most commonly performed procedures in the United States — and one of the most frequently unnecessary ones. Studies consistently show that for the most common types of lower back pain (disc herniations, spinal stenosis, degenerative disc disease), surgery produces outcomes no better than conservative treatment in the long run.

A landmark study published in Spine followed patients with herniated discs for ten years. Those who had surgery and those who didn't had virtually identical outcomes at the ten-year mark. The surgery group recovered faster initially — but the non-surgical group caught up, and both groups ended up in the same place.

Meanwhile, back surgery carries real risks: infection, nerve damage, failed back surgery syndrome (a condition where pain persists or worsens after surgery), and the need for additional surgeries down the road.

Before you consider surgery, it's worth exhausting every conservative option — especially ones that address the root cause of your pain.

Why Medication Doesn't Solve the Problem

Pain medication — whether over-the-counter NSAIDs or prescription opioids — can provide temporary relief, but it does nothing to address why your back hurts. It's the equivalent of removing the battery from a smoke alarm instead of putting out the fire.

Long-term NSAID use carries risks including gastrointestinal bleeding, kidney damage, and cardiovascular events. Opioids carry well-documented risks of dependence and addiction. Neither is a sustainable solution for chronic lower back pain.

What the Research Actually Supports

The evidence base for lower back pain treatment has shifted significantly in recent years. Major medical organizations including the American College of Physicians now recommend that most lower back pain be treated with non-pharmacological approaches first, including:

  • Movement and exercise — specifically targeted, individualized exercise
  • Mind-body approaches — addressing the stress and tension component of pain
  • Manual therapy — hands-on work to restore mobility
  • Postural correction — addressing the structural causes of pain

How Postural Alignment Addresses Lower Back Pain

The vast majority of lower back pain is mechanical — meaning it's caused by the way your body is loaded and moving, not by disease or structural damage.   • Anterior pelvic tilt — the pelvis tips forward, compressing the lumbar spine and overstretching the lower back muscles
  • Posterior pelvic tilt — the pelvis tips backward, flattening the lumbar curve and loading the discs unevenly
  • Hip rotation — one or both hips rotate inward or outward, creating asymmetrical loading on the spine
  • Weak glutes — when the glutes aren't doing their job, the lower back compensates and becomes chronically overworked
